Group 1-B, Tier One:
* ID required for persons 70 years old and older
* Ambulatory and outpatient health care personnel
* Schools of allied health students, residents and staff
* Home health service providers and recipients
* Dental providers and staff
ID required for persons 70 and older, and work badge or other forms
of ID that can verify place of employment or profession for other
groups.
REGISTRATION IS REQUIRED
Registration form, copy of ID, and copy of insurance card (insurance is not required)
